Job Title: Service Coordinator
Location: Cleveland- Heritage Oaks
Program: Rental Management
Education/Experience Qualifications:

  • Bachelor’s degree in social work, psychology or related field with at least 2 years’ experience in social service delivery with senior citizens and/or people with disabilities. Associates degree with significant experience may qualify.
  • Must have valid Tennessee driver’s license and proof of insurance on automobile.

Duties and Responsibilities:

  • Works with seniors and residents in apartment complex, located in Cleveland, to locate and refer to supportive services and other resources.
  • Assists residents to determine need for supportive services. Services could include transportation, meals, homemaker services, etc.
  • Educates residents on services available in the community.
  • Works with local service providers to determine available services in community and to make referrals.
  • Prepares and maintains confidential and secure client files which include intake form, information on referrals, case management plan, notes and other items.
  • Prepares and maintains records of program activities for reporting including data on residents served and types of services arranged for and received by residents.
  • Participates in area aging network and local community groups related to aging.

Bilingual Assistant/Translator – full-time, 32.5 hours/week, school year

  • Works closely with the MHCS Staff and the Executive Director of MCHS to provide services in a manner that flows well with their organization of appointment making, and providing them with the information they need better to serve each client.
  • Perform Case Management services with any willing participant through the CSBG Program.
  • Works closely with the CSBG Program Director regarding Commodities and recruits appropriate volunteers and other help as needed to conduct the commodities in Hamblen County.
  • Helps with monthly reports and all paperwork needed for the Commodity Program through DCEA for Hamblen County.
  • Conducts annual for the Commodity Program to obtain permission slips, bulk orders and all other necessary paperwork needed from participants.
